SCC Case Management

Syracuse Community Connections Inc. assesses the needs of families in the community to ensure that their needs are met. Case management helps those in the community identify their needs and set smart goals. Case management collects intake information from people who walk in the SCC and are not associated with any program. Needs are assessed and residents are referred to the appropriate program whether that service is within SCC or to a program in the community.  These referrals include but are not limited to: food assistance from the food pantry, clothing, and assistance with emergency shelter.
